Captured for A Cause -Artist Information

Please fill out the following form. Make sure that your name is how you would like it presented next to your image. For you title  put the type of artist you are, for example do you want your title to be "sculptor" or "clay sculptor"? "painter" or "oil painter"? Your bio should include how many years you have been doing this, other art you may do or something relative to your process or what make you unique. 

If you are donating a piece of work (or two) please list what will be auctioned off and a rough value for the piece. That way I can have an appropriate starting bid and value for the silent auction sheet.

1. There is no charge for photography services

2. I will choose the image for the exhibition out of the images you have approved

3. If no feedback on images is given by November 1, I will just pick an image

4. Any approved images can be used for promotion of this event on social media and on my website

5. Artists are encouraged but not obligated to donate to the silent auction

6. Artist are encouraged to attend the event

7. Artists are encouraged to help promote the event

8. All proceeds will be donated to Red Door Food Pantry in Cartersville, GA

9. No physical or digital images will be give to artists at no charge, but they may be purchased at a discounted price

10. Anyone under 18 must have parent a model release form

11. All images are owned by and copywrited to Lifesong Photography. By signing below you are giving consent for these images to be use on promotional materials for the Captured For A Cause 2026 event, to be displayed at the Captured For A Cause 2026 event and to be used by Lifesong Photography on their website, social media pages and entered into photographic competition.
